Student Change Form

If a current student employee is moved to a new level, receives a pay increase, or employment is extended, please fill out the student_employee_change_form_new.pdf and send via campus mail to APPO or OPERATIONS at the Parcade Office Building.

Multiple Assignment Form

A student may be employed at two on-campus positions if the student is currently working less then eighteen hours a week and/or the student possesses a highly specialized skill or knowledge that is needed by the second employing unit.  Should the following petition be approved, under no circumstances may a student work more than 18 (semester); 40 (during school breaks) total hours per week between the two on-campus positions.  Each student is required to have a completed Multiple Assignment Form AND Job Description Form for the second proposed position.  Please return these documents to Phyllis Enea in Student Services Building (SSB) 230.

Evaluation Form

This Evaluation Form is intended as a tool to assist the student employee and the supervisor in communicating any road blocks to the student’s performance and continued employment in the unit.
